Here you will find all the information and forms needed to make a statutory pay claim.
Please find below forms that can be downloaded to be completed and sent to the Staff Bank Payroll Team for processing, or guidance on how to claim statutory pay.
Statutory Sick Pay
As a bank worker, you are eligible for Statutory Sick Pay (SSP). The amount paid may vary year on year – for the most up-to-date information on the rates of pay, click here. You can claim Statutory Sick Pay for up to 28 weeks. When you are making a claim, the first three days will always be classified as a “waiting period” and will be unpaid. The only exception to this will be if you have already claimed SSP in the last eight weeks.
How to claim SSP
In order to claim SSP, you must send a doctor’s note or hospital note stating that you are not fit to work. The note must state the dates that you are deemed not fit to work. Please e-mail a full and clear copy of this note to imperial.staffbank.recruitment@nhs.net whilst retaining the original note, as this may be required at a later date. Please state in the subject of the e-mail that you are requesting SSP, and in the body of the e-mail, confirm your full name, your staff number, and the dates that you are claiming for. The Staff Bank Payroll Team will pass this onto the Trust to be paid.
Statutory Maternity / Paternity Pay
The below forms will need to be sent, along with a copy of your/your partner's MATB1 form to imperial.staffbank.payroll@nhs.net
